Levorphanol is an opioid medication used to treat severe pain. The medication has an abuse potential. In addition to acting on opioid receptors, it also acts on NMDA receptors. Typical doses include 2 mg by mouth or subcutaneous injection every 6 to 8 hours.

It is related to dextromethorphan, an antitussive which is not an opiate. Dextromethorphan is a salt of the methyl ether dextrorotatory isomer of levorphanol. Dextromethorphan is an opioid, but it is available without prescription because one has to overdose on it to feel its effects. Another common drug which is a synthetic opiate is loperamide HCl, otherwise known as Immodium A-D. It is similar to DXM because it treads on a side effect of a very weak synthetic opiate.
