Escobar first appeared in the pilot episode. He was the boss of that episode's patient Silvio Perez. Silvio and his brother come to Sean McNamara and Christian Troy to get a surgery for Silvio so he can be able to "get some ladies". Sean and Christian later find out that Silvio molested Escobar's 7 year old daughter and works for a drug dealer. Christian later goes to a "botox party" which turns out to be a trap because the hostess' boyfriend reveals himself to be Silvio's boss Escobar Gallardo. Escobar wants revenge on Silvio for stealing his daughter's innocence. Escobar then gives Christian botox and injects some into his crotch. Escobar calls Sean from Christian's cell phone and finds out where Silvio is getting his surgery performed. Escobar shows up in the operating room with his henchman, only to discover that Silvio's brother had already killed Silvio himself, because Silvio's brother had a small daughter himself. Afterwards, Sean and Christian tie pieces of ham to the body, and dispose of it in a swamp for an alligator to dispose of.

Escobar doesn't return until the final two episodes of the first season. He shows up when a patient comes to Macnamara/Troy that wants her breast implants removed. Upon removal Sean realizes they are filled with heroin. Escobar comes in and tells them that the patient had agreed to smuggle in a millions dollars worth of heroin in her breasts for him. He wants to know how soon she can have another procedure done. Escobar then uses Sean and Christian as part of a drug smuggling operation. In the season finale Liz Cruz is shot in the leg by one of Escobar's people. Sean wants to kill Escobar but Escobar makes a deal with him. He will let Sean and Christian go if they will give him a new face. He explains that he needs the new face to be able to get out of the country because he is on the FBI's 10 Most Wanted List. They give Escobar a new face but when at the airport he is arrested by the FBI because Sean and Christian gave him a face belonging to someone even higher on the list. It is assumed he went to prison. Escobar had one subsequent appearance in the show's second season finale in Sean's dream sequence.
